We regret to announce that Jiz Jordan’s final day with CPPFM will be November 15. We know you share our sadness as you have grown to trust her dedication to serving you well. As many are aware, with her return to a part time schedule this summer, she and her husband are enjoying the blessing of their first child, Noah. We wish them the best as they are moving to be closer to their extended family.

We have a wonderful team of healthcare providers available and we are hiring a replacement provider to continue care ensuring the transition to another one of our providers is seamless. We will be reaching out to reschedule appointments as necessary to maintain the continuity of your care. Rest assured, we will do everything possible to ensure your absolute satisfaction during this transition.

I did a lot of research of doctors offices around my area and I’m glad I did! This office was definitely worth all the 5 star reviews they’ve gotten!! I requested Jiz Jordan as my doctor because I wanted a women doc! And let me tell you... I’ve never been to a doctor that was so sincerely concerned with making my feel as comfortable as they could! She listened to ALL my concerns with intent to believe me and help me. She was probably the sweetest most knowledgeable doctors I have ever worked with and I feel I’ve hit the jackpot!

PA Jiz Jordan, was the ultimate professional and showed compassion about my concerns and my overall health. I went in for my annual wellness check a couple days ago, the staff was amazing they were punctual and kind which in my experience is not the norm these days. It felt good not being treated like a number but an actual human being….
